TRT and Fertility: What Men Need to Know Before Starting Testosterone
TRT and Fertility: What Men Need to Know Before Starting Testosterone
There's one question every man of reproductive age should be asked before starting testosterone:
Do you want children in the future?
Because testosterone replacement therapy and fertility have an important relationship that isn't always understood.
Testosterone Can Reduce Sperm Production
It sounds contradictory.
If testosterone is the primary male sex hormone, wouldn't taking testosterone increase fertility?
Not necessarily.
When testosterone comes from outside the body, the brain detects that circulating androgen levels are adequate. Hormonal signaling involved in stimulating the testes can subsequently decrease.
The result?
The testes may produce less testosterone internally—and sperm production can decline.
“But I'm Not Trying to Have Kids Right Now.”
That's exactly why the conversation should happen before treatment.
Maybe children aren't part of your plans this year.
What about three years from now?
Five?
Ten?
Fertility goals should be part of treatment planning for men considering TRT, particularly younger patients.
Does This Mean Younger Men Can't Address Low Testosterone?
Absolutely not.
It means we need to determine why testosterone is low and discuss reproductive goals before deciding on treatment.
Depending on the cause of testosterone deficiency and the man's fertility goals, referral or alternative management strategies may be appropriate.
The key is having that conversation before treatment changes sperm production—not afterward.
Men's Hormone Health Requires Individualized Care
This is exactly why we don't want Hodag HydrO2 Men's Health to become a place where every man with fatigue walks out with testosterone.
Hormone management requires context.
Your age matters.
Your symptoms matter.
Your medical history matters.
Your laboratory results matter.
And your future plans matter.

Educational information only. Men who are concerned about fertility should discuss reproductive goals with their healthcare provider before beginning testosterone therapy.
